Your Role:

• Develop Individualized Care Plans to support patient recovery and mobility goals.

• Provide Patient & Family Education to encourage continued progress and independence.

• Collaborate with the Healthcare Team to coordinate quality, patient-centered care.

• Monitor and Document Patient Progress to ensure effective and goal-oriented outcomes.

Qualifications:

• Licensure: Active Physical Therapy license in the State of Arizona.

• Skills: Strong communication and interpersonal abilities to foster trust and motivation.

• Approach: Patient-centered, compassionate, and adaptable care mindset.

• Flexibility: Willingness to travel locally and make a meaningful impact every day.
