What You’ll Do as a Care Coordinator

As a Care Coordinator, you’ll play a vital role in ensuring the well-being and success of our residents. Your responsibilities will include:

• Developing and maintaining individualized care plans tailored to meet the unique needs of each resident.
• Coordinating with staff, families, and external service providers to ensure seamless care and support.
• Monitoring residents’ progress and adjusting care plans as needed to achieve desired outcomes.
• Maintaining accurate and up-to-date documentation of all care activities and communications.
• Advocating for residents’ needs and ensuring their voices are heard.
• Supporting a positive and safe living environment that encourages growth and independence.

What We’re Looking For

To thrive in this role, you’ll need:

• At least 1 year of experience in a similar role or in providing care services.
• Strong organizational and communication skills to effectively coordinate care and collaborate with others.
• A compassionate and empathetic approach to working with individuals from diverse backgrounds.
• The ability to adapt to changing circumstances and problem-solve effectively.
• A commitment to upholding the values and mission of Bridge 2 Success Group Home.
