As a Medical Care Coordinator, you will play a crucial role in ensuring individuals receive necessary medical services, attend appointments, and stay consistent with prescribed medications.
Key Responsibilities
- Assist clients in accessing ongoing services, including medical, dental, vision, housing, financial assistance, and legal advocacy.
- Support clients in obtaining or reactivating disability benefits, Medicaid, and other needed resources.
- Ensure timely medication distribution and support medication compliance.
- Conduct daily medication checks and document compliance in the EMR system.
- Monitor new or updated medications to ensure appropriate documentation and continuity.
- Complete and upload all incidental funds paperwork with accuracy and adherence to agency policies.
- Assist counselors with discharge planning.
- Perform additional duties as assigned.
Required Qualifications
- High School Diploma required; bachelor’s degree preferred.
- 1 year of Care Coordinator experience.
- 2 years of Behavioral Health experience.
- Experience working with individuals with substance use disorders preferred.
- Strong communication and problem‑solving skills.
- Ability to work effectively with a diverse population.
- Computer literacy and comfort with EMR systems.
