Position Overview:
We are seeking an Occupational Therapist to provide skilled therapy services in a skilled nursing facility. The OT will focus on improving residents’ independence with activities of daily living (ADLs), functional mobility, and safety to support successful rehabilitation outcomes.
Key Responsibilities:
Conduct comprehensive OT evaluations and reassessments
Develop and implement individualized treatment plans
Address ADLs including dressing, bathing, toileting, and transfers
Recommend adaptive equipment and compensatory strategies
Collaborate with nursing, therapy, and interdisciplinary teams
Maintain accurate and timely clinical documentation
Qualifications:
Active California Occupational Therapist license
SNF or post-acute experience preferred
Strong clinical reasoning and communication skills
