We are a small, purpose-driven medical practice led by a board-certified geriatrician, and we are looking for an NP or PA who is smart, compassionate, reliable, and genuinely enjoys caring for seniors.

This is not a high-volume, “hurry up and see as many patients as possible” position. We believe smaller caseloads lead to better care. Our providers have more time to get to know patients, communicate with families and facility staff, review medications thoughtfully, and create medical plans that actually make sense for each person.

What You’ll Be Doing

You will provide on-site medical care to residents in Assisted Living Facilities and Independent Senior Living Apartments across Middle Tennessee.

Responsibilities may include:

  • Seeing patients for routine and acute visits (admission visits with a comprehensive care plan are done by our geriatrician)

  • Managing chronic medical conditions

  • Reviewing and simplifying medications when clinically appropriate

  • Communicating with families and facility staff

  • Coordinating with pharmacies, labs, home health, hospice, and specialists

  • Helping create thoughtful geriatric medical plans based on each patient’s goals and needs

  • Documenting visits in the medical record

Locations may include:

  • Hendersonville

  • Nashville

  • Brentwood

  • Green Hills

  • Franklin

  • Bellevue

  • Antioch

  • Other Middle Tennessee locations as opportunities arise

Part-time or full-time options may be available.

Compensation & Perks

  • $35–$50 per completed follow-up visit, depending on experience and role

  • Paid twice monthly

  • Optional phone call coverage: $500 per pay period / $1,000 per month

  • $1 per minute for non-visit work on non-visit days (often an additional $500 - $1000/month)

  • Malpractice insurance available if needed

  • Flexible schedule

What You’ll Need

  • Active Tennessee NP or PA license

  • Active DEA license (if you are a new graduate and need a supervising physician first, we can work on that)

  • Laptop and smartphone

  • Reliable transportation

  • Strong communication skills

  • Good follow-through and documentation habits

  • A heart for older adults

Experience in long-term care, assisted living, geriatrics, primary care, hospice, or palliative care is a plus, but it is not required. If you love seniors and are willing to learn, we will support you with training and geriatric expertise.

Position Type

This is a 1099 independent contractor position, so traditional benefits are not included.
