Available Shifts (2 Positions Open)


  • Position 1 (Weekday): Monday – Friday, 8-hour day shifts.
  • Position 2 (Weekend): Friday – Monday.
  • Note: On-call rotation may be required.

Position Highlights


  • Manageable Workload: Balanced caseload of 5 to 6 patient visits per day (Routine, Recertification, Deaths, On-Call, and PRN visits).
  • Local Service Radius: Travel within an established 60-mile radius from the local branch.
  • EHR Platform: HomeCare HomeBase (HCHB) strongly preferred for efficient, stream-lined mobile documentation.

What You Will Be Doing


  • Comprehensive Assessments: Perform initial and ongoing clinical evaluations, establishing and updating individualized Plans of Care in collaboration with physicians.
  • Direct Care & Symptom Management: Administer skilled direct nursing interventions, specializing in pain relief, symptom control, and end-of-life hospice care.
  • Interdisciplinary Leadership: Oversee and guide care teams (LPNs, CNAs, support staff) to deliver all-inclusive physical, emotional, and psychosocial support.
  • Caregiver Education: Empower patients and family members through clear instruction on medication management, wound dressings, oxygen safety, and hospice philosophy.
  • Physician Collaboration: Serve as the primary liaison between physicians, patients, and the care team, swiftly reporting condition updates to attending providers.
  • Clinical Documentation: Maintain accurate, timely electronic medical records (visit notes, assessments, orders, and interdisciplinary notes).
  • Team Supervision & QAPI: Conduct monthly supervisory visits with care team members, perform regular clinical record audits, and participate in Quality Assessment and Performance Improvement (QAPI) initiatives.

Education & Credentials

• Active, unencumbered Registered Nurse (RN) license in the state of practice.


• Graduate of an accredited nursing program.


• Current American Heart Association (AHA) BLS certification.

Experience & Skills

• Minimum 1 year of RN experience (acute care hospital, oncology, LTC, hospice, or home health). Clinical hours/internships evaluated case-by-case.


• Proficiency in physical assessments, infection control, and case management.


• Strong communication, organizational skills, and ability to work independently.

Field Logistics

• Valid driver’s license and current automobile insurance.


• Reliable personal transportation to cover the 60-mile travel radius.

Compliance

• Ability to pass background checks, health screenings, and meet safety-sensitive requirements.

Desired Qualifications


  • Home Health EHR: Hands-on experience using HomeCare HomeBase (HCHB) software strongly preferred.
  • Specialized Background: Prior experience in Medicare home health, hospice, palliative care, or geriatric case management.
  • Clinical Expertise: Deep understanding of terminal care, managed care principles, regulatory guidelines (Medicare, ACHC), and symptom control.
