Position Overview
We are seeking a motivated and compassionate Case Manager (QMHA) to join our growing team. This role focuses on supporting clients in accessing resources, building stability, and navigating systems of care.
This is a full-time position, consisting of:
%CF; 25–30 client contacts per week
%CF; Additional administrative/documentation time

Key Responsibilities
%CF; Provide case management services in alignment with QMHA scope
%CF; Connect clients with community resources (housing, medical care, benefits, etc.)
%CF; Collaborate with clinicians and multidisciplinary team members
%CF; Support clients in achieving treatment plan goals
%CF; Maintain accurate, timely documentation in the EHR
%CF; Advocate for clients and reduce barriers to care
%CF; Conduct outreach when appropriate (community-based work may be included)

Qualifications
%CF; QMHA (Qualified Mental Health Associate) certification required (or ability to obtain upon hire, depending on candidate)
%CF; Bachelor’s degree in social work, psychology, or related field (or equivalent experience aligned with QMHA requirements)
%CF; Experience in behavioral health, social services, or community-based work preferred
%CF; Strong organizational and communication skills
%CF; Ability to work independently while collaborating with a team
%CF; Knowledge of community resources is a plus
