Nurse Practitioner - Geriatric Primary Care | Mobile/Hybrid
Job Description
Physician Senior Services, PLLC is seeking an experienced and compassionate Nurse Practitioner to join our growing geriatric primary care practice serving patients throughout the Dallas-Fort Worth area.
Our practice provides medical care primarily to older adults living in assisted living, memory care, and senior living communities. Rather than working in a traditional clinic setting, our providers travel to the communities we serve and see patients in their residential environment.
Providers typically round in assigned senior living communities during the morning hours, with afternoons generally reserved for charting, messages, orders, and care coordination that can be completed from home.
We are open to full-time, part-time, and independent contractor arrangements and are looking for a provider who enjoys caring for older adults, values autonomy, and is comfortable managing a medically complex patient population.
Position Responsibilities
Responsibilities include, but are not limited to:
- Providing primary care to patients in assisted living, memory care, and senior living communities
- Performing comprehensive assessments, routine follow-up visits, and acute-care visits
- Managing chronic medical conditions and age-related health concerns
- Reviewing and managing medications, laboratory results, imaging, and diagnostic testing
- Ordering appropriate laboratory testing, imaging, therapies, referrals, and other services
- Coordinating care with families, community staff, specialists, home health agencies, hospice providers, pharmacies, and other members of the healthcare team
- Evaluating changes in condition and determining appropriate treatment or level of care
- Completing timely and accurate documentation within the electronic medical record
- Participating in ongoing communication with the practice's clinical and administrative support team
- Participating in the practice's shared provider on-call rotation
- Providing patient-centered care with an emphasis on quality of life, safety, and appropriate medical decision-making
Practice Model
This is a mobile/house-call provider position, not a traditional office-based clinic role.
Providers travel between assigned senior living communities within their geographic territory, primarily during the morning hours. Afternoons are generally used for chart completion, messages, orders, follow-up, and care coordination and may typically be completed from home.
Our in-office clinical team assists with scheduling, messages, records, orders, referrals, medication requests, and care coordination, allowing providers to remain focused on patient care.
The position offers a significant amount of independence while still providing the support and resources of an established medical practice.
Employment & Compensation Options
We recognize that experienced Nurse Practitioners may have different preferences regarding compensation and employment structure, and we are open to discussing an arrangement that works for both the provider and the practice.
Opportunities may include:
- Full-Time W-2 Employee: $115,000–$140,000 annually, commensurate with experience and qualifications, plus employee benefits
- 1099 Independent Contractor: compensation structured as an agreed-upon percentage of collected professional revenue, with earning potential based on patient volume and productivity
- Part-Time arrangements may also be considered depending on availability and practice needs
Specific compensation, productivity expectations, scheduling, and employment structure will be discussed individually with qualified candidates.
Schedule & On-Call
- Primarily Monday through Friday, daytime hours
- Morning rounding in assigned senior living communities
- Afternoon charting, messages, orders, and care coordination generally completed from home
- No routine overnight shifts
- Participation in a shared provider on-call rotation, approximately one week every five weeks
Qualifications
- Current and unrestricted Texas Nurse Practitioner license
- National board certification as a Nurse Practitioner
- Current Texas prescriptive authority
- DEA registration or ability to obtain
- Current BLS certification
- Valid driver's license and reliable transportation
- Strong clinical judgment and communication skills
- Ability to work independently and manage a patient panel efficiently
- Comfortable using an electronic medical record and mobile technology
Preferred Experience
Experience in one or more of the following areas is preferred:
- Geriatrics
- Primary care
- Internal medicine
- Family medicine
- Assisted living or memory care
- Home-based primary care
- Skilled nursing or long-term care
- Hospice or palliative care
Candidates with strong primary care experience who are interested in transitioning into geriatric medicine are also encouraged to apply.
