Clinical Pharmacy Specialist - Oncology
Job Description
Jamaica Hospital Medical Center a renowned Queens, New York Community based hospital and Level 1 Treatment Center currently seeks an Oncology Clinical Pharmacy Specialist for an immediate full-time day shift opportunity.
Our ideal candidate will have a minimum Board Certification in Oncology (BCOP), with at least 4 years of experience in Oncology management. A PGY 1 Residency and a PGY2 residency in Oncology is preferred, or equal clinical experience in Oncology with BCOP.
Selected candidate will provide clinical pharmacy services for inpatient and outpatient hematology/oncology medication management; and be responsible for the management of adult patients receiving ambulatory chemotherapy as well as patients with oncology related admissions. He/she will actively participate in committee work and projects to advance and elevate pharmacy practice and investigational drug services related to oncology studies.
JOB RESPONSIBILITIES
• Collaborating with inter-professional team to enhance communication, coordinate care, and improve patient satisfaction.
• Prospectively evaluating all drug therapy for appropriate indications, dosage, drug interactions, and drug allergies; monitors the patients’ pharmacotherapeutic regimen for effectiveness and ADEs and intervenes as needed.
• Identifying ADEs and assisting in their management and prevention, reporting and developing process improvements to reduce drug errors and preventable ADEs.
• Providing pharmacokinetic monitoring when a targeted drug is prescribed.
• Coordinating with stakeholders to ensure timely access to medications.
• Partnering with patients to determine the most appropriate sources for their medication needs and referring patients to appropriate programs and resources.
• Assisting in insurance related issues and working with coordinators of prior medication authorizations.
• Providing drug therapy related education to Oncology team members.
• Documenting clinical activities that include, but not limited to, disease state management, general pharmacotherapeutic monitoring, pharmacokinetic monitoring, ADEs, education, and other patient care activities.
• Providing didactic lectures to health professional residents and students in oncology pharmacology and therapeutics, where applicable
• Acts as a liaison between pharmacy, nursing, and the medical staff to educate health professionals regarding current drug-related procedures, policies, guidelines, and pathways.
• Developing drug monographs on issues related to chemotherapeutic and supportive care medications.
• Implementing and maintaining departmental policies and procedures related to safe and effective use of drugs.
• Collaborating with nursing, medical staff, and hospital administration to prepare for Joint Commission surveys and responding to any deficiencies identified.
• Providing consultation to hospital committees, such as Pharmacy Therapeutics and Nutrition, when oncology pharmacotherapy issues are discussed.
• Identifying how drug costs may be minimized through appropriate use of chemotherapeutic medications and through implementation of cost-containment measures.
• Participating in quality assurance programs to enhance pharmaceutical care.
• Developing order sets and treatment plans within the Electronic Medical Record and the Clinical Oncology Application
• Precepting student pharmacists interested in an Oncology rotation.
• Participating in investigational drug services related to oncology studies, as needed.
QUALIFICATIONS
· Doctor of Pharmacy Degree from an accredited College of Pharmacy or equivalent
· Board Certification in Pharmacotherapy (BCPS) or Board-Certified Oncology Pharmacist (BCOP) preferred.
· Completion of PGY1 Pharmacy Practice residency and 3-6 years oncology experience preferred.
· PGY2 Specialty residency in Adult Oncology or equivalent Adult Oncology experience and minimum 2 years of clinical experience preferred.
· New York State Pharmacist License, or eligibility to sit for the exam.
SALARY: $150K - $170K DOE Plus, Fully Benefits & PTO
