Job Description
Tranquility Hospice and Palliative Care is seeking compassionate team players to join our hospice nursing team. We have full and part-time openings for Registered Nurse Case Managers and Admission Nurses. Weeknight and weekend on-call rotation. IN licensure required. Hospice experienced preferred.
Summary of essential job functions:
Serves as the primary care nurse for all assigned cases and provides skilled nursing services.
Observe, evaluate and report to the attending physician and interdisciplinary team patient's physical and emotional conditions and reactions to drugs or treatments.
Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible.
Reassess pain and notifies the primary care physician when pain is not manageable on current medication.
Develops a patient care plan utilizing the written physician's orders, communicates and interprets the goals of care to the patient and family.
Completes written documentation for the medical record, assist the family to accept the responsibilities for providing care in the home and provides emotional support for these activities.
Supervises hospice aides.
Requirements:
Must be an active Registered Nurse (RN) for the State of Indiana.
Minimum of 1 years' experience, preferably in the area of Hospice.
Prolonged or considerable walking or standing.
Able to lift, position, or transfer patients.
Able to lift supplies and equipment.
Considerable reaching, stooping, bending, kneeling, or crouching.
Visual acuity and hearing to perform required nursing skills.
