Registered Nurse Home Health Case Manager

The Registered Nurse Home Health Case Manager plays a vital role in delivering comprehensive nursing care to patients in their home environments. This position involves assessing patient needs, managing medications, educating patients and caregivers, and coordinating multidisciplinary care within a team-based caseload. The nurse will utilize an electronic health record system to document care and communicate effectively with all stakeholders to ensure optimal patient outcomes.

Responsibilities

• Conduct thorough patient assessments during in-home visits
• Manage and monitor patient medication regimens
• Educate patients and caregivers on disease management and health maintenance
• Coordinate care among healthcare providers, specialists, and community resources
• Maintain accurate and timely documentation using electronic health record systems
• Communicate effectively with patients, families, and team members
• Collaborate with a multidisciplinary, team-based caseload

Preferred Qualifications

• Minimum of 1+ years experience in nursing
• Active Registered Nurse (RN) license
• Proficiency in patient assessment and care coordination
• Experience with electronic health record (EHR) systems
• Background in home health care and medication management
• Excellent communication, time management, and problem-solving skills
