About the Opportunity
InHome Home Health is seeking a highly qualified and experienced Registered Nurse (RN) – Home Health Manager to join our organization in Indianapolis, Indiana. This leadership role is responsible for directing home health nursing services, ensuring the delivery of high-quality patient care, and supporting clinical excellence across the care team. The ideal candidate will bring strong clinical expertise, sound leadership capabilities, and a commitment to advancing positive patient outcomes in a home health setting.
Key Responsibilities
- Provide leadership and oversight of home health nursing operations to ensure the delivery of high-quality, patient-centered care.
- Conduct comprehensive patient assessments and develop individualized care plans aligned with clinical needs and established standards of care.
- Supervise, coach, and support nursing staff while ensuring compliance with applicable healthcare regulations, policies, and procedures.
- Collaborate with physicians, case managers, and interdisciplinary team members to coordinate care and optimize patient outcomes.
- Monitor patient status and adjust care plans as appropriate to support effective and timely treatment interventions.
- Ensure accurate, timely, and complete clinical documentation in accordance with regulatory and organizational requirements.
- Provide education to patients and families regarding health conditions, treatment plans, and home care management strategies.
- Participate in performance improvement and quality assurance initiatives to enhance clinical operations and service delivery.
Qualifications
- Current and active Registered Nurse (RN) license in the state of Indiana.
- OASIS experience preferred not mandatory.
- Minimum of 3 years of nursing experience, preferably in home health, community health, or a related clinical setting.
- Proven leadership and supervisory experience, with the ability to manage teams and promote a culture of accountability and excellence.
- Strong verbal and written communication skills, with the ability to engage effectively with patients, families, clinicians, and other stakeholders.
- Demonstrated critical thinking, clinical judgment, and decision-making abilities in a fast-paced healthcare environment.
- Working knowledge of home health regulations, compliance requirements, and industry best practices.
- Current CPR and First Aid certification required.
- Proficiency in electronic health records (EHR) and clinical documentation platforms.
