Case Manager / Service Coordinator

As a case manager you will work with clients and their families to coordinate resources and services to meet the client's needs. The case manager may work in a variety of settings, including hospitals, long-term care facilities, government agencies, and social service organizations.

As a case manager, you are responsible for assessing the needs of the clients and developing individualized plans. The case manager will also collaborate with other professionals, service providers, and organizations to coordinate care and ensure that clients receive the most appropriate services. Case managers are also responsible for documenting progress, communicating with clients and family members, and advocating for their clients.
