PRN Family Nurse Practitioner

Position Summary Continental Wellness Center is seeking a compassionate and experienced Family Nurse Practitioner (FNP) to provide comprehensive primary care services with a focus on HIV prevention, PrEP management, and care for individuals living with HIV/AIDS . As a Ryan White service provider , our organization delivers integrated medical and behavioral health services to underserved communities. The ideal candidate will have experience working with HIV-positive patients, individuals at high risk for HIV, and patients with co-occurring mental health conditions.

Key Responsibilities

%CF; Provide primary care services to patients, including health assessments, diagnosis, and treatment planning.

%CF; Manage and prescribe HIV Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP) programs.

%CF; Conduct HIV testing, prevention counseling, and risk-reduction education.

%CF; Provide medical care and ongoing management for patients living with HIV/AIDS .

%CF; Provide medical case management health assessments and service plans

%CF; Monitor lab results, medication adherence, and patient outcomes.

%CF; Coordinate care with mental health providers, case managers, and support services.

%CF; Maintain compliance with Ryan White program requirements and clinical documentation standards.

%CF; Participate in quality improvement initiatives and public health reporting.

%CF; Educate patients on prevention, treatment adherence, and overall wellness.

%CF; Collaborate with community partners to support HIV prevention initiatives.

Qualifications

%CF; Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)

%CF; Active Florida APRN license

%CF; Board certification as Family Nurse Practitioner

%CF; DEA registration

%CF; Experience with HIV care, infectious disease, or PrEP programs preferred

%CF; Experience working with underserved populations

%CF; Knowledge of Ryan White Program standards preferred Skills

%CF; Strong patient-centered communication

%CF; Knowledge of HIV prevention and treatment guidelines

%CF; Cultural competency and trauma-informed care

%CF; Electronic medical record (EMR) proficiency

%CF; Ability to work in an integrated care team

%CF; Bilingual preferred
