Job duties for children’s ICM will include:
-Assessing individuals, developing individual care plans and determining necessary resources.
-Advocating for and assisting individuals and families in accessing and obtaining services.
-Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals.
-Monitoring individual rehabilitation goals and progress.
-Providing crisis intervention services.
Travel is required. Individual will be provided with a state vehicle to use.
Minimum staffing requirements, including licenses and certifications: Bachelor's in Social Work degree, working under the supervision of a licensed master social worker (LMSW) or licensed clinical social worker or New York State LCSW or LCSWs. Valid drivers license.
Schedule and anticipate length of assignment: Monday through Friday, 8:00am - 4:30pm, anticipated to last 6 months, but is not guaranteed.
OMH required training: Training will be provided by OMH.
