Job Description

At Ace Care Management, you’ll support members throughout their care journey while working in a structured, team-driven environment. This is a hybrid role that includes fieldwork and remote responsibilities and requires a minimum of 2 in-office days per week for collaboration and case coordination.
Key Responsibilities
- Manage and oversee assigned members’ care plans
- Conduct monthly phone contacts and quarterly in-person visits
- Advocate for members’ clinical and service needs
- Coordinate communication between members and providers
- Monitor compliance with approved service plans
- Recommend care plan modifications as needed
- Maintain thorough and accurate documentation
- Participate in collaborative weekly team meetings
Requirements
- Bachelor’s Degree (Social Work, Human Services, or related field preferred)
- 2+ years of experience in case management, healthcare, or human services
- Strong organizational skills and ability to manage multiple priorities
- Comfortable with field work, documentation, and a hybrid work environment
- Valid driver’s license and reliable transportation
