Direct Support Professional/ Caregiver- Group Home

• Provide direct care assistance with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility support
• Assist members with meal preparation, feeding, and following prescribed diets
• Administer medications and complete Medication Administration Records (MARs) in compliance with DDD and agency policy
• Follow each member’s Individual Support Plan , Behavior Support Plan , and medical protocols
• Monitor member health and safety, report changes in condition, and document incidents accurately and timely.
• Maintain a clean, safe, and structured home environment
• Transport members to medical appointments, day programs, work, and community activities as authorized
• Implement positive behavior supports and de-escalation techniques as trained
• Complete required documentation, including daily notes, shift reports, incident reports, and logs.
• Work collaboratively with supervisors, healthcare providers, families, and support coordinators
