RN Case Manager

RN Case Manager – Bemidji, MN

We're looking for a compassionate and independent Registered Nurse Case Manager to join our team in Bemidji. In this role, you'll coordinate care for clients in their homes, develop and manage care plans, provide clinical oversight, and serve as an advocate for your patients and their families.

Responsibilities:
• Develop and manage individualized care plans
• Conduct client assessments and ongoing follow-up
• Coordinate care with physicians, caregivers, and other healthcare professionals
• Provide clinical supervision and training as needed
• Monitor client satisfaction and advocate for additional services when needed
• Ensure accurate and timely documentation
• Build relationships within the local healthcare community

Qualifications:
• Active RN license
• Strong communication and organizational skills
• Ability to work independently and manage multiple priorities
• Commitment to providing high-quality, patient-centered care
• Home care experience preferred but not required
