Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans for home health patients
- Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting
- Monitor and evaluate patient progress, adjusting care plans as needed in coordination with the physician
- Educate patients and family members on disease management, medications, and home safety
- Accurately document patient care, clinical notes, and visit records in compliance with agency and regulatory standards
- Collaborate with a multidisciplinary care team including therapists, social workers, and physicians
- Respond to patient needs and changes in condition, escalating care when appropriate
- Active Florida RN license in good standing
- Previous home health, med-surg, or community nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Familiarity with OASIS documentation and Medicare/Medicaid home health guidelines a plus
- Valid Florida driver's license, auto insurance, and reliable personal transportation
- Excellent communication and patient education skills
- Ability to work independently and manage a patient caseload with minimal supervision
- CPR/BLS certification required
