Field Assessment Nurse (LPN/RN)
Job Description
Position Summary
The Field Assessment Nurse is responsible for conducting comprehensive in-person nursing
assessments to support the initiation and continuation of client services. The nurse is expected to
complete all assigned assessments accurately, timely, and in their entirety while maintaining
compliance with agency policies, state regulations, and professional nursing standards. The
position requires effective communication, strong organizational skills, and collaboration with
clients, caregivers, providers, and interdisciplinary team members to ensure quality service
delivery and continuity of care.
Primary Responsibilities
The Field Assessment Nurse is responsible for, but not limited to:
- Complete all assigned initial, reassessment, annual, and ongoing nursing assessment within required timeframes.
- Conduct thorough telephonic assessments and obtain all required clinical information.
- Accurately complete all assessment documentation in its entirety.
- Document all client interactions and work performed in accordance with agency requirements.
- Communicate professionally with clients, family members, caregivers, providers, agency staff, and other involved parties throughout the assessment process.
- Monitor and manage assigned assessment portfolio to ensure timely completion of all required nursing assessments.
- Follow up on outstanding information or documentation necessary to complete assessments.
- Maintain compliance with agency policies, program requirements, HIPAA regulations, and applicable nursing standards.
- Attend all required departmental meetings, clinical meetings, trainings, and staff meetings.
- Maintain clear, timely, and professional communication with supervisors and team members regarding assessment status, barriers, or concerns.
- Perform other related duties as assigned in support of agency operations.
Compensation Clarification
Compensation is based on completed assessments. Pay ranges from $85-$145 per successful completion of assessments.
