Hospice Clinical Liaison

Job Summary:

We are seeking a compassionate, driven and experienced Hospice Community Liaison to join our healthcare team in the Coachella Valley / Greater Palm Springs area. In this business development role, you will serve as the face of our organization. Your primary role is to facilitate seamless transitions of care for patients requiring end-of-life care through clinical assessments, education, and timely admissions coordination while building business relationships with local hospitals, skilled nursing facilities, assisted living facilities, physicians and community organizations.

Key Responsibilities:

• Relationship Management - Develop and maintain strategic partnerships with case managers, discharge planners, physicians, healthcare administrators and managers, and other potential referral sources
• Clinical Assessments - Conduct pre-admission clinical assessments to determine hospice eligibility in accordance with Medicare guidelines
• Patient and Family Education - Meet with patients and families to explain hospice benefits, address patient and family concerns, and guide them through the intake and admission process
• Community Outreach - Prepare, coordinate and conduct in-service training sessions, community information workshops and similar activities to foster community education on end-of-life care and to promote widespread understanding of hospice options for families and providers
• Collaboration - Coordinate closely with the admissions team to ensure a smooth patient transition
• Compliance - Ensure compliance with HIPAA, Medicare and CDPH regulations, and ethical referral practices

Qualifications:

• Clinical background preferred (RN / LVN/ Social Worker/ Chaplain)
• At least One (1) year experience in hospice , home health or healthcare sales/ marketing
• Strong understanding of Medicare hospice benefit, as well as other Federal and State regulations relating to hospice
• Excellent communication, presentation and relationship-building skills
• Compassionate, organized and a self-starter with the ability to create own leads
