Compensation
- $90,000 - $110,000 Per Year
- $500 annual credit for CEUs

About the Role
We are seeking a dedicated Home Health Physical Therapist to deliver comprehensive, one-on-one rehabilitative care to patients directly within the comfort of their homes. In this role, you will design and execute customized treatment plans focusing on gait training, balance improvement, fall prevention, and pain management to restore functional mobility. Operating with a high degree of scheduling autonomy, you will work closely with patients and their families to build overall well-being and promote safe, healthy, and independent living.
Physical Requirements & Professional Credentials
- Physical Effort: Requires moderate physical effort, including walking, bending, stretching, stooping, pushing, and occasional heavy lifting.
- Work Environment: Performed in a fast-paced patient care environment requiring strong prioritization skills; occasional exposure to seasonal environmental conditions or cramped workspaces.
- Clinical Safety: Occasional exposure to serious injury, environmental hazards, infectious diseases, bio-hazardous materials, blood/bodily fluids (requiring standard precautions), and combative patients.
- Licensure: Must maintain a current, valid state Occupational Therapy license in good standing and an active CPR card.
- Attendance: Must maintain reliable, consistent attendance to ensure safe, continuous patient care.
