Intake Coordinator

• Conduct comprehensive psychosocial assessments and identify clients' mental health, emotional, social, and environmental needs.
• Develop individualized treatment plans based on assessment findings and client goals.
• Provide individual, family, couples, and/or group counseling and psychotherapy.
• Diagnose and treat mental and emotional disorders within the scope of licensure, training, and professional competency.
• Provide crisis intervention, supportive counseling, and problem-solving interventions as appropriate.
• Monitor client progress and modify treatment plans when clinically indicated.
• Maintain accurate, timely, and confidential clinical documentation in accordance with agency, ethical, and regulatory requirements.
• Coordinate care with physicians, psychiatrists, counselors, case managers, schools, community agencies, and other providers.
• Make appropriate referrals and connect clients with community resources and supportive services.
• Advocate for clients and assist with access to healthcare, housing, financial assistance, and other needed resources.
• Participate in interdisciplinary treatment teams and case consultations.
• Maintain professional boundaries and comply with applicable ethical and legal standards.
• Participate in quality improvement, program development, and evaluation activities as assigned.
• Provide consultation and education to clients, families, staff, and community partners.
• When appropriately licensed and designated, provide clinical supervision to other social workers and related behavioral health professionals.
