PRINCIPAL DUTIES AND RESPONSIBILITIES:

1. Adheres to UCHC policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct.

2. Makes Customer Service a priority, treating customers (residents, visitors, and co-workers) in a professional manner exercising courtesy and tact. Effectively communicates with residents and others about UCHC services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in UCHC in a timely fashion.

3. Proficient in the use of computer for basic entry of scheduling and planning information.

4. Supervise Physical Therapist Assistants, Aides, students and volunteers as indicated in Union Community Health Center..

5. Screens individuals to determine need for physical therapy. Performs necessary evaluation procedures and records results in accordance with standards of practice.

6. Develop treatment plans, including long and short term goals, and methods to achieve identified goals. Coordinate treatment plan with patient, family and/or primary caretaker, and Rehab team.

7. Implement plan directly or supervise treatment by a Physical Therapist Assistant.8. Collaborate with members of multidisciplinary team to integrate P.T. treatment with total patient care plan.

9. Documents results of evaluations, treatment plans, progress and discharge from services in a clear and concise manner and in accordance with established standards.

10. Completes all required documentation for departmental purposes in a timely manner.

11. Develops appropriate home programs to maintain/enhance the performance of patients in their own environment. Properly documents and follows up with these programs.

12. Maintains daily caseload of ascribed number of patients.

13. Evaluates need for splints, ambulation devices and/or adaptive equipment as indicated. Recommends or issues equipment necessary to promote patient’s functional independence.

14. Maintains department facilities and equipment according to environmental safety and infection control guidelines as set forth by the Hospital, Nursing Home, UCHC-OPD and Department of Health.

15. Contributes to ongoing program development by providing in-service contribution of resources. Attends all staff in-services and meetings.

16. Performs all other duties as directed by supervisor(s) / Director.

17. Abides by the professional standards set forth by the New York State Board of Professional Services, JCAHO, APTA and other accrediting agencies.

18. Attends continuing education conferences related to profession and departmental needs.

19. Promote a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c).

20. Organizes/manages time and work schedule effectively and efficiently and maintaining departmental productivity standar

QUALIFICATIONS:

A. Education Required: Graduate of an accredited Physical Therapy program.

B. License(s) Required: New York State license and current registration.

C. Experience/Skills Required: Good interpersonal skills.

WORKING CONDITIONS: Direct patient contact; physically very active; frequent standing and considerable moving about; lifting and/or carrying of materials related to treatment procedures; patient transfers; assisting patients with ambulation during gait training.
