We aim to provide local patient assignments whenever possible.

What You'll Do

As a Home Health Physical Therapist, you will:

• Perform comprehensive PT evaluations in patients' homes.
• Develop individualized treatment plans based on each patient's needs and goals.
• Provide therapeutic exercises, gait training, balance training, and mobility interventions.
• Help patients improve strength, function, mobility, and independence.
• Provide fall prevention and safety education.
• Teach patients and caregivers appropriate home exercise programs and safe mobility techniques.
• Communicate and collaborate with physicians, nurses, OTs, SLPs, caregivers, and other members of the care team.
• Monitor patient progress and adjust treatment plans as needed.
• Complete timely and accurate electronic documentation.

What We're Looking For

• Active Colorado Physical Therapist License
• Graduate of an accredited Physical Therapy program
• Strong communication and patient education skills
• Ability to work independently and manage your schedule effectively
• Valid driver's license and reliable transportation
• Compassionate, professional, and patient-centered approach

Home Health experience is preferred—but not required.
