Essex Home Health is hiring a Full‑Time LPN to provide home‑health visits for approximately 40 patients per week, focusing on hypertension and diabetic care.
Key Duties
- Complete home‑health nursing visits (40 patients weekly)
- Provide hypertension and diabetic patient care
- Perform assessments, vitals, medication teaching
- Document visits accurately and timely
- Communicate patient changes to RN Case Manager
- Work every other weekend
- Travel between Lawrence, Haverhill, and Methuen (mileage reimbursement)
Requirements
- Active LPN license in Massachusetts
- Home‑health experience preferred
- Strong communication and patient‑education skills
- Reliable, organized, and able to work independently
