LPN Licensed Practical Nurse

We are seeking a motivated, self-starter LPN for our Trenton Location! In this role you will be responsible for the medical oversight and care for all service recipients at our 16-bed group home facility.

Responsibilities:

• Medication Oversight and Auditing
• Medical Appointment Transportation and Coordination
• Ensuring that all PCP orders are followed
• Updating Treatment Plans with weekly progress notes
• Day to Day Management of facility
• Supervision and Coaching of Employees
• Oversight of Service recipient interactions with family and community
• Provide timely and appropriate feedback and response to upper management

Requirements:

• Licensed nurse in Tennessee
• Pass a drug screen and background check
