The service coordinator position is responsible for service planning, monitoring, assessing and identifying needs and goals of consumers diagnosed with intellectual and developmental disabilities relating to personal outcomes, including recommendations for interventions and support services. Arranges and collaborates with service providers and community agencies in order to link consumers with outcome-focused services. The service coordinator will be responsible for managing a caseload not to exceed forty-five (45) individuals. The caseload will consist of consumers who utilize waiver funding streams to include, HCS and TXHML. The Service Coordinator is responsible for making contacts with assigned consumers in accordance to their service coordination assessment. Service coordinators are responsible for completing an annual person-directed plan (PDP) for each person on their caseload and, completing timely and accurate documentation of the delivery of service coordination services. Requires flexibility in scheduling and significant travel within the service area. The job calls for a self-motivated and energetic person to assess consumer’s outcomes and needs and provide monitoring of services to consumers living in their natural homes, and/or foster homes. The caseload will be concentrated primarily in Atascosa, Frio, and Wilson Counties, however depending on agency need; this position might be assigned individuals residing in other surrounding counties. Service Coordinators are required to meet set productivity standards on a monthly basis. This position will be housed at the Maverick CRU Admin office in Eagle Pass, TX, and will be directly supervised by the IDD Waiver Programs Director.


Minimum Qualifications:

  • Bachelor’s degree in social work, psychology, human services, or a related field
  • Knowledge of community resources.
  • Strong organizational and communication skills.

Preferred Qualifications:

  • Experience working with individuals with intellectual and developmental disabilities.
  • Experience with person-centered planning and trauma-informed care.
  • Bilingual abilities or experience working with diverse populations.
