Job Overview:```
We are seeking a skilled and compassionate Licensed Vocational Nurse to join our healthcare team. As an LVN, you will play a crucial role in providing quality care to clients with Developmental disability in the home setting. ```Duties:```
- Administer medications and treatments as prescribed by physicians
- Monitor patients' vital signs and report any changes or abnormalities to the RN and MD.
- Assist with medical procedures and examinations
- Maintain accurate and up-to-date medical records
- Provide emotional support and education to patients and their families
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Coordinate scheduling of clients for consultant and medical appointments.
- Accompany clients to their scheduled medical appointments.
- Assist with discharge planning and ensure smooth transitions for patients
```Qualifications:```
- Valid LVN license in the state of California
- Experience working with adults in the home care settings.
- Knowledge of medical terminology, physiology, and standard nursing practices
- Familiarity with sterile processing techniques
- Strong communication and interpersonal skills
- Ability to work well in a fast-paced environment
- Excellent attention to detail and organizational skills
