Compensation: $25.00 – $30.00 / hour

Work Location: Field-based / On the Road (Company vehicle provided)
Lead Field Care Logistics, Quality Assurance, and Account Growth
We are a licensed and bonded in-home care agency seeking a proactive, organized Care Coordinator to manage field operations, oversee service quality, and drive local business growth.
In this role, you will be on the road ensuring seamless care delivery—monitoring client care plans, evaluating caregiver performance, assisting with client onboarding, and expanding our reach through field sales and professional referral development.
Core Responsibilities
Care Logistics & Field Operations:
- Conduct regular on-site quality assurance visits with existing clients to review care plans and ensure service expectations are met.
- Monitor, observe, and evaluate caregiver performance, reliability, and protocol adherence in the field.
- Assist senior care management with client intake, care preparations, and onboarding as needed.
- Serve as the primary field liaison between client families, caregivers, and the office team to resolve scheduling or operational issues quickly.
- Identify evolving client needs, updating care requirements and recommending expanded service hours when appropriate.
Field Sales & Business Development:
- Prospect, visit, and build active referral relationships with local businesses, healthcare providers, rehabilitation centers, and senior facilities.
- Present and promote in-home care solutions to prospective clients and community partners to secure new accounts.
- Deliver value-add wellness activities (such as basic blood pressure and weight checks) to engage potential clients and generate qualified leads.
- Maintain pipeline follow-up to transition inquiries into active, satisfied clients.

Qualifications
- Background in home care, caregiving, healthcare operations, or field customer service (CNA, HCA, or care coordination experience strongly preferred).
- Proven capability in managing care logistics, auditing care plans, and evaluating field personnel.
- Solid communication and relationship-building skills to handle prospective client outreach and sales pitching.
- Valid driver’s license with an insurable, clean driving record.
