We are looking for a committed, motivated, forward thinking and excellent time managed Physical Therapist to act as the patient’s partner throughout their journey of restoring motion/flexibility/mobility and ensuring that the client will function at their personal best.

Responsibilities

  • Evaluate patients with a variety of diagnoses, establish treatment plans and formulate goals specific to the patient.
  • Perform therapeutic procedures, exercise, and modalities indicated by patient’s condition and functional status.
  • Test, evaluate and record initially and sequentially, patient status so that objectives are determined, treatment plan developed and regularly reviewed/revised.
  • Instruct patients in home programs for the purpose of advancing the patient's independence in self-care and activities of daily living.
  • Record patient's progress so ensure the best multidisciplinary approach to improve the patient's current functional level; report any significant changes in patient's condition so that the treatment plan may be modified.
  • Assists in discharge planning.
  • Document comprehensive daily notes, patient evaluations, progress notes, re-evaluations, discharge summaries and other patient care coordination activities via EHR software.
  • Attend scheduled departmental meetings, training and team conferences.
  • Coordinate with other departments to meet the needs of our patients.
  • Follow all practice safety protocols including sanitization practices.
  • Assist with other responsibilities as needed.

NOTE: This is for a part time position for the following days/shifts : Monday, Wednesday and Friday 3pm-7pm.
