As we continue to grow, we are seeking a skilled and motivated Physical Therapist to join our team and help improve patient outcomes across San Bernardino, Riverside, and Orange County.
Responsibilities:
Perform in-home evaluations and develop individualized care plans
Provide physical therapy services in accordance with each patient's plan of care
Educate patients and caregivers on exercises, mobility strategies, and safety
Collaborate with physicians, nurses, and other clinicians to ensure coordinated care
Complete timely and accurate clinical documentation
Qualifications:
Current and valid California Physical Therapist license
Minimum 1 year of experience (home health preferred, but not required)
Reliable transportation and a valid driver’s license
Strong communication, clinical judgment, and time management skills
