Registered Nurse/Delegating Nurse & Case Manager
Job Description
We are seeking a Registered Nurse/Delegating Nurse/Case Manager to join our team! Come work in one of the most rewarding fields in nursing. We currently have a position to work with adults with intellectual disabilities at our Hagerstown, MD location. Position is eligible for full time benefits. Total of 40hrs per week, M-F day shift.
Responsibilities:
- Assessments for assigned caseload
- Development of and training for Nursing Care Plans
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Delegate and serve as resource to Direct Support Professionals/Certified Medication Technicians
- Educate patients/Direct support professionals about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Valid MD Nursing License or Compact State License
- Previous experience with intellectual/developmental disabilities preferred but not required
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a case management environment
