Case Manager – Temporary Position
Location: New York, NY
Schedule: Monday–Friday, 4:00 PM–12:00 AM
Pay: $22.00 per hour

Position Overview
We are seeking a Case Manager to provide comprehensive, person-centered, strengths-based services to individuals living in the community.
The Case Manager will manage a caseload of approximately 15 individuals and provide support designed to help clients live full, independent, and satisfying lives in the community.
This position is ideal for a compassionate and dependable professional who is committed to advocacy, client engagement, and helping individuals access the services and resources they need.
Key Responsibilities
- Provide comprehensive case management services to an assigned caseload of approximately 15 individuals.
- Provide person-centered and strengths-based support based on each individual's needs and goals.
- Assist clients with financial assistance and entitlement benefits.
- Provide counseling, advocacy, referrals, and information.
- Provide crisis intervention and support as needed.
- Engage clients through regular outreach and ongoing communication.
- Monitor client needs, progress, and service plans.
- Provide psycho-education and supportive services.
- Escort clients to appointments and other services within the community when needed.
- Coordinate care with outside service providers and community resources.
- Collaborate with colleagues and team members to address client needs and support program goals.
- Maintain required documentation and meet program and contractual requirements.
- Participate in weekly supervision.
- Participate in agency-provided training and professional development opportunities.
- Perform other duties as assigned.
Qualifications
- Experience providing case management, social services, behavioral health, or related support services preferred.
- Strong interpersonal and communication skills.
- Ability to build trusting relationships with individuals from diverse backgrounds.
- Strong advocacy and problem-solving skills.
- Ability to work independently while also functioning effectively as part of a multidisciplinary team.
- Strong organizational skills and attention to detail.
- Ability to manage multiple responsibilities and priorities.
- Comfortable working with individuals who may have complex or varying support needs.
- Reliable and dependable.
- Strong commitment to person-centered and strengths-based service delivery.
