Palliative & Hospice Consultants PLLC is seeking a compassionate and experienced Nurse Practitioner to join our growing inpatient Palliative Care team at DMC Sinai-Grace Hospital in Detroit, Michigan.

Position Highlights

Schedule

  • Full-time schedule consisting of 12 consecutive working days followed by 2 consecutive days off
  • Rotating weekend coverage (every other weekend)

  • Starting salary from $130,000 annually, based on experience and qualifications

Patient Practice Environment

  • Manage an average daily census of approximately 30 patients within a structured inpatient practice.
  • Practice in a collaborative hospital environment dedicated to providing high-quality palliative care services.

Primary Responsibilities

  • Conduct comprehensive palliative care consultations and follow-up evaluations.
  • Assess and manage pain, symptoms, and other complex medical conditions associated with serious illness.
  • Develop individualized treatment plans in collaboration with the supervising physician.
  • Lead goals-of-care conversations and facilitate advance care planning discussions.
  • Educate patients and families regarding serious illness, treatment options, symptom management, and available support services.
  • Collaborate with physicians, nurses, case managers, social workers, chaplains, therapists, and other interdisciplinary healthcare professionals.
  • Participate in multidisciplinary rounds to support coordinated, patient-centered care.
  • Prescribe and manage medications in accordance with Michigan laws and organizational policies.
  • Maintain timely, accurate, and complete documentation within the electronic medical record.
  • Contribute to the continued growth and success of the hospital's palliative care program through clinical excellence, education, collaboration, and compassionate patient care.
  • Perform other duties as assigned.

Qualifications

Required

  • Graduate of an accredited Nurse Practitioner program (MSN or DNP)
  • Active and unrestricted Michigan Nurse Practitioner license
  • Current DEA registration
  • Minimum of three (3) years of Nurse Practitioner experience
  • Current BLS and ACLS certification
  • Eligible for credentialing and privileging at DMC Sinai-Grace Hospital
  • Excellent communication, interpersonal, and clinical decision-making skills

Preferred

  • Experience in Palliative Care, Hospice, Hospital Medicine, Internal Medicine, Critical Care, Oncology, or Geriatrics
  • Hospice and Palliative Care certification or equivalent experience
  • Experience leading family meetings and goals-of-care discussions
  • Passion for interdisciplinary collaboration and patient-centered care
