RN: Case Manager Compensation

• Pay rate: $57.44
• Shift Requirements: Mon-Fri 8am-5pm, no on-call.

RN: Case Manager Responsibilities:

• Perform comprehensive assessment of high risk members.
• Collaborate with primary care providers to ensure the implementation of individualized care plans.
• Address and continuously reassess appropriate levels of care.
• Coordinate care transitions and work with multiple stakeholders.
• Engage specialty resources as needed to achieve optimal outcomes.
• Keep detailed records of clinical, functional, and fiscal outcomes.
• Act as patient advocate while protecting privacy and confidentiality issues.

RN: Case Manager QUALIFICATIONS:

Critical Information

• Shift Requirements: Mon-Fri 8am-5pm, no on-call.
• Onsite position.
• Counties covered: Orange, Durham, Alamance.

Education/Licenses Needed

• Associates or Bachelor’s Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license.
• Case Management certification.
