What You'll Do

As part of the SURVE Wellness care team, you may:

• Provide one-on-one assistance with bathing, dressing, grooming, toileting, and personal hygiene
• Assist with walking, transfers, mobility, and range-of-motion activities
• Provide companionship and meaningful interaction
• Prepare meals and assist with eating and hydration
• Perform light housekeeping related to the patient's care
• Provide non-clinical medication reminders
• Observe and report changes in the patient's condition
• Complete required documentation accurately and on time
• Follow the patient's individualized plan of care
• Help maintain a safe, comfortable, and respectful home environment
• Communicate and collaborate with the SURVE clinical team

Who We're Looking For

We welcome applicants who meet at least one of the following:

• Florida Home Health Aide (HHA) certification
• Active Florida Certified Nursing Assistant (CNA) license
• Previous professional caregiving or home care experience

We'd Especially Like to Meet Candidates Who Are

• Experienced in home health or private-duty care
• Dependable, compassionate, and professional
• Comfortable working independently in patients' homes
• Strong communicators with excellent interpersonal skills
• CPR certified
• Able to provide reliable transportation
• Available for flexible schedules
• Bilingual in English/Spanish or English/Creole
