LPN Home Health

We are seeking a Home Health LPN to join our team. You will deliver skilled nursing care to patients in their residences, helping them maintain health, recover from illness, and manage chronic conditions. This is a great opportunity to join our established home health agency.

We offer premium per diem or visit-based compensation

Responsibilities

• Provide direct patient care according to physician orders and care plans.
• Monitor and record patients' vital signs and overall health status.
• Administer medications and treatments as prescribed.
• Perform wound care, dressing changes, and catheter care.
• Observe patients for changes in condition and report findings to the supervising RN or physician.
• Educate patients and caregivers on disease management, medications, and home care procedures.
• Maintain accurate and timely patient documentation.
• Assist with activities of daily living (ADLs) when appropriate.

Required Qualifications

• Current and valid LPN license (State of Michigan) is required
• Experience in home health or community nursing is preferred
• Valid driver's license, reliable transportation, and automobile insurance.
• Previous home health, long-term care, or clinical nursing experience preferred.
• Strong communication, assessment, and organizational skills.
