Salary: $75,000–$85,000 per year
Schedule: Full-Time, Monday–Friday, On-Site
Position Overview
We are seeking a Clinical Coordinator II to provide clinical leadership and supervision within a supportive housing environment serving older adults and individuals with histories of homelessness, mental health challenges, and/or substance use.
The Clinical Coordinator II will supervise social work and case management staff while maintaining a small caseload and providing direct clinical support to residents. This position plays an important role in ensuring high-quality services, regulatory compliance, staff development, and positive outcomes for residents.
The ideal candidate is an experienced social work professional with strong clinical and supervisory skills who is comfortable working in a diverse residential setting.
Key Responsibilities
- Provide clinical and case management supervision to social work and case management staff.
- Provide clinical guidance and support to staff as needed.
- Maintain a small caseload and develop strong, supportive relationships with residents.
- Participate in the review of applications for admission and assist with admission decisions.
- Participate in home visits with prospective applicants as needed.
- Provide clinical support and resources during psychiatric emergencies.
- Review and approve support plans, case management summaries, case notes, psychosocial histories, and other required documentation.
- Conduct regular chart audits to ensure documentation is complete, timely, accurate, and compliant with contractual and regulatory requirements.
- Meet regularly with the Building Manager regarding resident and building-related concerns.
- Collaborate with building leadership regarding staff training and provide resident-related counseling and guidance to building staff.
- Lead weekly team meetings with social work staff.
- Facilitate regular meetings with building leadership and social work staff.
- Provide orientation, training, and ongoing support to new social work staff.
- Review incidents and incident reports and ensure required documentation is submitted appropriately.
- Participate in site audits and assist with developing and submitting Plans of Correction when required.
- Supervise social work interns as needed.
- Assist residents with obtaining and maintaining appropriate medical and mental health services.
- Assist residents with obtaining and maintaining benefits and entitlements.
- Help residents establish and maintain family and social connections.
- Assist residents in identifying appropriate recreational and socialization opportunities.
- Complete required documentation related to resident discharges and transfers.
- Provide crisis intervention and supportive counseling.
- Visit residents in hospitals, nursing homes, and other care facilities as needed.
- Facilitate community-building activities, including groups, resident activities, and holiday events.
- Perform other duties as assigned.
Required Qualifications
- LMSW required.
- Minimum of 3 years of experience working with older adults and/or individuals with histories of homelessness, mental health challenges, and/or substance use.
- Strong clinical and case management skills.
- Experience providing supervision or clinical oversight preferred.
- Strong understanding of person-centered and strengths-based service delivery.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds.
- Ability to work collaboratively with social workers, case managers, building management, outside providers, and other professionals.
- Strong documentation and record-keeping skills.
- Ability to respond effectively to crises and changing resident needs.
- Ability to communicate effectively in English, both verbally and in writing.
Preferred Qualifications
- At least 2 years of supervisory experience preferred.
- Experience working in supportive housing, affordable housing, behavioral health, homeless services, or senior services.
- Experience supervising social work or case management staff.
- Experience working with older adults and/or formerly homeless populations.
