POSITION: INTAKE AND THERAPY COORDINATOR FOR HOME HEALTH

REPORTS TO: CLINICAL NURSING SUPERVISOR AND ASST. ADMINISTRATOR

DEPARTMENT: ADMINISTRATIVE

DUTIES AND RESPONSIBILITIES:

· Receive, organize, and input patient data into the Home Health software when referrals come in

· Verifying health insurance coverage related to home health qualification and determine agency’s acceptance of new patients/referrals based on agency’s policies.

· Fielding calls and emails from patients and providers

· Maintain good communication with referral sources, patients, families, communities regarding patient care and schedules.

· Scheduling visits, consultations, and other services

· Resolve simple to moderately complex issues, corrects errors and refers complex issues to supervisor.

· Maintain a tracking sheet of pending orders.

· Ensure that documentation and required orders are signed by clinicians and saved appropriately in EMR software.

· Take and upload orders for labs. Carry out, review and revise orders for X-ray and therapy.

· Track labs, order phlebotomist and communicate results to Nurses, Physicians, fax results to Physicians and Pharmacies

· Order medical supplies

· Coordinate and schedule therapy visits with therapy companies

· Ensure therapy visits are documented appropriately and in a timely manner on EMR systems.

· Review new admission paperwork for appropriate MD visit note and F2F documentation.

· Assist with ordering all mobile diagnostic services.

· Create QA notes and other identified areas of need to assist with QA process.

· Upload new orders for current patients and assist field staff as needed for implementation.

· Serve as Quality Assurance resource to assist field staff.

· Ensure patient files and notes are reviewed and ready to be submitted for billing.

· Take patient phone calls as needed to support nursing needs.

· Assist with filling, faxing, and answering phones as needed.

· Schedule follow-up skilled nursing visits on assigned days/times during work week.

· Keep organized with clinicians and service areas for coverage.

· Manage nursing and therapy visit-schedules, to ensure accurate on EMR software at all times.

· Manage patient documentation and tasks to ensure accurate on EMR software at all times.

· Follow-up with clinicians for documentation submission in a timely manner

· Communicate effectively with biller and coder.

· Maintain confidentiality of all agency records

· Ensure that agency policies and procedures are being followed according to accreditation body and federal/state regulations.

· Manage payroll data and update business office on information needed to

· Maintain HR paperwork for newly hired and existing clinicians.

· Hire and recruit clinicians needed based on agency’s staffing needs.

· Performing other clerical work

· Other responsibilities as assigned.

· On the on-call rotation

REQUIREMENTS

  • At least one year of intake experience in Home Health preferred or 1 year or more of hands-on nursing experience

  • Customer interaction/customer service experience

  • Strong knowledge of Microsoft Office along with strong data entry skills

  • Excellent organizational, analytical, communication and customer service skills

  • Ability to take initiative and problem solve

  • Excellent attention to detail and follow-up skills

  • Experience working to obtain insurance verification, initiate pre-authorizations, and enter insurance data correctly to ensure the billing process runs smoothly

  • Working knowledge of Medicare, VA, commercial, private pay providers and regulations that may impact the verification of insurance for the Agency

  • Prior experience in a medical field.
