About the Role:
As a Physical Therapist with our team, you will be responsible for providing high-quality physical therapy services to our patients, including assessment, treatment, and education. You will work closely with our interdisciplinary team to design and implement individualized plans of care that meet the unique needs of each patient. With a focus on patient-centered care, you will prioritize communication, collaboration, and compassion in all aspects of your work.
Responsibilities:
Conduct initial and ongoing assessments of patients to identify physical therapy needs and develop individualized plans of care
Design and implement evidence-based treatment plans to achieve specific goals and outcomes
Provide physical therapy services, including but not limited to, exercises, modalities, and manual therapy
Collaborate with the interdisciplinary team to develop and implement comprehensive plans of care
Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions and ongoing care
Maintain accurate and timely records of patient care and treatment
Requirements:
Current state licensure as a Physical Therapist
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced, dynamic environment
Willingness to work varying schedules, including days, evenings, and weekends
