Brinton Manor Nursing and Rehabilitation Center is seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team. In this role, you will evaluate residents, develop individualized treatment plans, and provide evidence-based therapy to help patients regain mobility, improve strength and balance, reduce pain, and maximize their independence.

ResponsibilitiesEvaluate residents and develop individualized physical therapy treatment plans.
Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
Monitor resident progress and modify treatment plans as needed to achieve optimal outcomes.
Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other interdisciplinary team members.
Educate residents, families, and caregivers on treatment plans, exercises, and injury prevention strategies.
Accurately document evaluations, treatments, progress notes, and discharge summaries in accordance with facility policies and regulatory requirements.
Maintain compliance with all state, federal, and professional standards of practice.
Participate in quality improvement initiatives to enhance resident outcomes and satisfaction.

Current Physical Therapist (PT) license in the state of Pennsylvania.
Graduate of an accredited Physical Therapy program.
Previous experience in a skilled nursing facility, rehabilitation center, or long-term care setting is preferred but not required.
Strong clinical assessment and treatment planning skills.
Excellent communication, documentation, and interpersonal skills.
Compassionate, patient-centered approach to care.
