Brinton Manor Nursing and Rehabilitation Center is seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team. In this role, you will evaluate residents, develop individualized treatment plans, and provide evidence-based therapy to help patients regain mobility, improve strength and balance, reduce pain, and maximize their independence.

Responsibilities
  • Evaluate residents and develop individualized physical therapy treatment plans.

  • Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.

  • Monitor resident progress and modify treatment plans as needed to achieve optimal outcomes.

  • Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other interdisciplinary team members.

  • Educate residents, families, and caregivers on treatment plans, exercises, and injury prevention strategies.

  • Accurately document evaluations, treatments, progress notes, and discharge summaries in accordance with facility policies and regulatory requirements.

  • Maintain compliance with all state, federal, and professional standards of practice.

  • Participate in quality improvement initiatives to enhance resident outcomes and satisfaction.

Qualifications
  • Current Physical Therapist (PT) license in the state of Pennsylvania.

  • Graduate of an accredited Physical Therapy program.

  • Previous experience in a skilled nursing facility, rehabilitation center, or long-term care setting is preferred but not required.

  • Strong clinical assessment and treatment planning skills.

  • Excellent communication, documentation, and interpersonal skills.

  • Compassionate, patient-centered approach to care.

  • Ability to work effectively as part of an interdisciplinary healthcare team
