We are seeking an experienced Intake Coordinator who can confidently manage the intake process, provide excellent customer service, and ensure a smooth and professional experience for every client.
⭐ KEY REQUIREMENTS? Fluent in English is REQUIRED — strong verbal and written communication skills are essential for this position.
? Spanish-speaking is PREFERRED — candidates who can communicate effectively with Spanish-speaking clients and families are highly encouraged to apply.
? Previous Intake or Healthcare Coordination experience is REQUIRED.
We are looking for someone who can step into the role with confidence and understands the importance of timely follow-up, accurate documentation, organization, and exceptional customer service.
? What You'll DoServe as a primary point of contact for new clients, families, referral sources, and healthcare professionals.
Conduct initial intake calls and gather accurate client information.
Communicate with clients and families regarding services, intake procedures, and next steps.
Communicate with Spanish-speaking clients and families when applicable.
Explain available services and guide clients through the intake process.
Coordinate assessments, appointments, interviews, and admissions.
Collect, review, and track required documentation.
Follow up with clients and referral sources regarding pending information and requirements.
Maintain accurate and organized client records.
Coordinate with internal teams to ensure a smooth transition from referral to service.
Monitor pending referrals and ensure timely follow-up.
Handle phone calls, emails, SMS, and other forms of communication professionally.
Address client and family questions or concerns and escalate issues when appropriate.
Maintain confidentiality and follow agency policies and applicable healthcare regulations.

Provide a welcoming, professional, and compassionate experience to every client from the first interaction.
Previous experience in intake, healthcare coordination, home care, admissions, case management, or a related healthcare position.
Fluent in English.
Excellent verbal and written communication skills.
Strong organizational and time-management skills.
Excellent follow-up and attention to detail.
Comfortable handling multiple calls, referrals, and priorities.
Professional, dependable, and compassionate.
Strong computer and documentation skills.
Spanish-speaking skills — highly preferred.
Experience working in home care, healthcare, behavioral health, or services for individuals with disabilities.
Experience with intake or electronic healthcare/client management systems.
