The Lakes Home Care is looking for a motivated, organized, and detail-oriented Intake Coordinator to join our growing team!
Responsibilities:
• Coordinate new patient referrals and admissions
• Communicate with physicians, hospitals, and referral sources
• Verify patient information and required documentation
• Schedule and coordinate start of care
• Work closely with our clinical team to ensure a smooth intake process
• Maintain accurate records and provide excellent customer service
Qualifications:
• Strong communication and organizational skills
• Ability to multitask in a fast-paced environment
• Experience in home health or healthcare is preferred but not required
• Proficiency with computers and Microsoft Office
