Position Summary

Under the direction of the Health Center Director, the Referral Coordinator is responsible for coordinating patient appointments, specialty services, referrals, transportation, and registration processes. This position supports the efficient coordination of care for patients at the correctional health center while maintaining a professional, confidential, and patient-centered environment.

The Referral Coordinator uses sound judgment, initiative, and strong communication skills to ensure patients receive timely access to appropriate healthcare services. The position may also assist with training staff on appointment and scheduling procedures as needed.

Key Responsibilities

• Schedule appointments for Unity Health Care patients from a central location.
• Schedule specialty appointments for patients.
• Perform patient registration functions, including insurance verification.
• Review patient-related forms for accuracy and completeness and assist patients when necessary.
• Coordinate the appointment process for correctional patients.
• Arrange transportation to outside medical providers and diagnostic testing appointments.
• Assist with obtaining specialist consultation reports.
• Maintain a clean, organized, and safe working environment.
• Maintain patient files and/or databases.
• Answer telephone calls professionally and ensure messages are communicated to providers in a timely manner.
• Assist with training staff on appointment and scheduling processes as needed.
• Maintain a professional and confidential working environment.
• Perform other duties and responsibilities as assigned.

Minimum Qualifications

• High school diploma or equivalent.
• Minimum of three (3) years of experience in an office setting, preferably in a medical office environment.
• Strong customer service skills.
• Knowledge of medical procedures and insurance processes.
• Excellent oral, written, and telephone communication skills.
• Bilingual English/Spanish preferred.
• Ability to work independently and effectively within a team-oriented environment.
• Ability to work professionally and effectively with individuals from diverse ethnic and cultural backgrounds and with varying needs.
