Part Time | 11p-7a, EOW

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Eden Nursing & Rehabilitation Center (formally known as Platinum Ridge) in Brackenridge, Pennsylvania. About the Role:

As a Licensed Practical Nurse (LPN), you will be responsible for providing high-quality, patient-centered care to our residents. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, Monitor and report changes in resident conditions, and maintain accurate and up-to-date records. You will also be expected to maintain a clean and safe environment, respond to emergencies, and participate in quality improvement initiatives.

Responsibilities:

  • Provide direct patient care to residents, including administration of medications, treatments, and therapies

  • Develop and implement individualized care plans, taking into account the unique needs and preferences of each resident

  • Monitor and report changes in resident conditions, including but not limited to, changes in vital signs, behavior, and physical condition

  • Maintain accurate and up-to-date records of resident care, including but not limited to, medical records, medication administration records, and incident reports

  • Participate in quality improvement initiatives to enhance the quality of care and services provided to residents

  • Maintain a clean and safe environment, including but not limited to, the cleanliness of resident rooms, common areas, and equipment

  • Respond to emergencies, including but not limited to, fires, spills, and medical emergencies

  • Participate in in-service training programs and continuing education opportunities to stay current with industry trends and best practices
