We are seeking a compassionate and skilled Physical Therapist (PT) to join our dedicated team at Rivers Edge Nursing & Rehab in Muscoda, Wisconsin. As a valuable member of our rehabilitation team, you will play a vital role in helping our patients achieve their goals and regain independence.
About the Role:
As a Physical Therapist with Rivers Edge Nursing & Rehab, you will have the opportunity to work with a diverse range of patients, from acute care to long-term care, in a state-of-the-art rehabilitation environment. Physical Therapist (PT) responsibilities will include:
Conducting comprehensive patient assessments and developing individualized treatment plans
Providing physical therapy interventions, including exercises, stretches, and modalities, to help patients achieve their goals
Collaborating with other healthcare professionals to ensure seamless care coordination and effective patient outcomes
Maintaining accurate and detailed patient records, including progress notes and discharge summaries
Participating in staffing meetings and contributing to a positive and supportive work environment
Requirements:
Physical Therapist licensure in the state of Wisconsin
Physical Therapist degree from an accredited program
Physical Therapist experience in acute care, long-term care, or a combination of both
Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
