As a Community Liaison, you will play a vital role in facilitating communication and collaboration between our organization and the community we serve.

About the Role:

As a Community Liaison, you will serve as the primary point of contact for our organization, developing and maintaining strong relationships with community leaders, healthcare providers, and other stakeholders. Your exceptional interpersonal and communication skills will enable you to build trust and foster partnerships that support our mission to deliver high-quality, patient-centered care.

Responsibilities:

  • Develop and maintain relationships with community leaders, healthcare providers, and other stakeholders to promote our organization's services and build partnerships

  • Identify and address community needs, developing strategies to increase awareness and utilization of our services

  • Provide education and support to patients, families, and healthcare providers on our services and resources

  • Participate in community events and networks to promote our organization and enhance our reputation

  • Collaborate with our care team to ensure seamless transitions and coordination of patient care

  • Develop and maintain accurate records and reports to track community outreach and engagement activities

Requirements:

  • Bachelor's degree in a related field (e.g., social work, public health, healthcare administration)

  • Prior experience in community outreach, patient engagement, or healthcare administration

  • Excellent communication, interpersonal, and problem-solving skills

  • Ability to work independently and as part of a team

  • Strong organizational and time management skills

  • Familiarity with hospice care and the needs of terminally ill patients and their families
