We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our skilled nursing facility team!


The Vineyards Healthcare Center is a skilled nursing facility located at 76 Fenton Street, Livermore, California. It offers 24-hour Medicare-certified care in an 83-bed facility near downtown Livermore. It has received high-performing ratings in both short-term rehabilitation and long-term care from the U.S. News & World Report. The center is committed to providing individualized, holistic care through warm and meaningful interactions, aiming to help residents achieve their best outcomes and ensure their comfort is a top priority.

RESPONSIBILITIES:

  • Administering medications and monitoring patient reactions

  • Assisting with daily living activities such as bathing, grooming, and feeding

  • Maintaining accurate patient records and documentation

  • Collaborating with RNs and other healthcare team members to implement care plans

  • Conducting routine physical examinations and health assessments

  • Educating patients and families about health conditions and care plans

  • Ensuring compliance with safety and health regulations and protocols


QUALIFICATIONS:

  • Experience in a skilled nursing facility/long-term care/senior care is a plus

  • Valid and active Licensed Vocational Nurse (LVN) license in the state of California.

  • Must have an active BLS through the American Heart Association

  • Must have an active CPR Certification

  • Must be in good standing with all regulatory agencies and licensing boards

  • Interest in working with the geriatric population

  • Completion of an accredited vocational nursing program.

  • Knowledge of the nursing process and standards of care for skilled nursing facilities

  • Strong communication and interpersonal skills

  • Ability to prioritize and multitask in a fast-paced environment

  • Excellent critical thinking and problem-solving abilities

  • Basic computer skills and proficiency in electronic health records
