Reports To: Health Information Supervisor / Operations Manager

FLSA Status: Non-Exempt

Position Summary

The Medical Records Clerk is responsible for maintaining the integrity, confidentiality, and accuracy of client health records. This position prepares, organizes, scans, indexes, files, retrieves, and maintains electronic and paper medical records while ensuring compliance with HIPAA, Medi-Cal, DHCS, LACDMH, and agency standards. The Medical Records Clerk supports clinical and administrative staff by ensuring timely access to accurate health information.

Essential Duties and Responsibilities

· Prepare, organize, file, and maintain client medical records.

· Retrieve medical records for authorized clinical and administrative staff.

· Scan, index, and upload documents into the Electronic Health Record (EHR).

· Review records for completeness, accuracy, and required documentation.

· Process medical record requests in accordance with HIPAA and agency policies.

· Maintain strict confidentiality of protected health information.

· Assist with audits, quality assurance reviews, and record retention activities.

· Verify demographic and billing information for accuracy.

· Respond professionally to internal and external record requests.

· Perform other administrative duties as assigned.


Minimum Qualifications

· High School Diploma or equivalent required.

· Minimum one (1) year of medical records, health information, or medical office experience preferred.

· Knowledge of HIPAA confidentiality requirements.

· Experience with Electronic Health Records preferred.

· Valid California Driver License preferred.

Bilingual preferred


Knowledge, Skills & Abilities

· Excellent organizational and filing skills.

· Attention to detail and accuracy.

· Strong customer service and communication skills.

· Proficiency in Microsoft Office and data entry.

· Typing speed of at least 40 words per minute.

· Ability to manage multiple priorities in a fast-paced healthcare environment.
